MyChart - Patient Portal

You Are More Than a Patient at CCM Health

That’s why we provide you with easy online access to your patient information. By logging into our system using your unique ID and Password, you will securely be transferred to our MyChart where you can view your personal medical statistics, such as prescribed medications, upcoming appointments, and key metrics from your most recent visit. This is a private, online tool for you to manage your health. Your information can only be viewed by you. At CCM Health, we value your privacy rights.

Take control of your own health care using our easy-to-navigate patient tools. We hope the statistics and medical records you find on your MyChart page will be a powerful incentive to set good health goals for your future. You have access to this information wherever you have an internet connection.

MyChart is a secure, online tool connecting patients electronically to portions of their medical record.
To Access MyChart you will need computer access with an Internet connection, or a smart phone.
MyChart access is granted to users who have registered with their clinic or hospital. Access to information in MyChart is secured by a personalized ID and password, known only to the user.

To register for MyChart, sign up during a clinic or hospital visit, or online at mychart.centracare.com/MyChart/signup.

Once registered on MyChart, you will have access to the following information:

View your medical information online
• Review your medications, immunizations, allergies and medical history.
• Receive test results online — no waiting for a phone call or letter.
• Review health education topics and discharge instructions provided by your provider.

Stay in touch with your provider
• Communicating with your provider is as simple as sending an e-mail — but even more secure.
• Request prescription refills for any of your refillable medications.

Manage your appointments
• Schedule an appointment.
• View details of past and upcoming appointments.

Pay your bill online
• Make a payment online.
• View any outstanding balance.
• Send a message to the billing office.

Access your loved one’s records
• Link your loved one’s accounts to yours for convenient access to appointments, test results, immunization records, growth charts and more.
Children age 0-11, parent/guardian will be granted full access to the child’s MyChart record.
Children age 12-17, parent/guardian will be granted partial access to the child’s MyChart record (i.e. immunizations).
Children age 12-17 can register for their own MyChart access. When the child reaches age 18, parent/guardian will no longer have any access to the child’s MyChart record. Adult proxy will be granted full access to the MyChart record.

Fire Safety

Fire Prevention: Your Safety Comes First

A hospital is a place to care for people, and we take your safety very seriously. That is why we take extra measures to prevent fires from happening in and around the hospital building. If a fire does occur, our hospital is equipped with alarms that will detect the fire quickly. Plus, the building is designed to put down automatic smoke barriers to isolate individual areas and prevent the spread of fire and smoke.
If you do hear a fire alarm, you should remain where you are and wait for instructions from the staff in your unit. Our staff is trained to follow emergency procedures. Our goal is to always keep our patients, families, and staff safe and informed.

When a fire is reported in the facility, the Montevideo Fire Department is automatically alerted, and they will arrive as quickly as possible to deal with the situation.

What Can You Do to Stay Safe?

One of the most important ways to remain safe from a hospital fire is to prevent one from happening! Do not smoke when you are here. Our hospital and medical clinic are tobacco-free. That means you may not smoke anywhere on the campus. Smoking is a fire hazard under normal conditions, but in a hospital zone where there are many flammable substances, such as oxygen, smoking is a huge risk. If you see someone smoking on the hospital campus, please report it immediately to the nearest staff member.

Your Safety is Our Top Priority

We are so concerned about keeping our patients safe that we established a safety committee. The safety committee meets four times a year to discuss new implementations and policies to keep our patients and our facility safe. We are always striving to be better at preventing fires and other emergency situations. If you have any concerns about potentially unsafe situations while you are at CCM Health, please let a staff member know right away.

The Patient Self-Determination Act of 1990

The Patient Self-Determination Act of 1990 assures that each patient has a right to decide his or her own treatment. This includes:

  • the right to accept or refuse procedures and treatments
  • the right to make advance directives
  • the right to make organ donations

Our Policy

CCM Health respects and encourages patient self-determination. We make it possible for patients to be active participants in decision-making about their care through education and inquiry. Furthermore, we encourage patients to communicate their preferences and values regarding their medical care to loved ones. We believe that such communication will guide surrogates in medical decision-making for patients who lose their decision-making capacity.

Please note that CCM Health Hospital and Medical Clinics will not discriminate against nor require any condition for care based on whether the patient has executed an advance directive.

What is an Advance Directive?

An advance directive is a written, legal document that states your preferences for medical treatment in the event that you are unable to make the decision yourself. It can be a living will, which pertains to your medical treatment at the end of life. It can also be a statement that appoints someone to make health care decisions for you if you are no longer able to make your own decisions.

Pre-registration collects important information that allows us to better serve you, including health-history, insurance, and updates on your emotional, physical, and mental well-being. Completing the necessary paperwork beforehand helps you skip the convoluted check-in process so we can get straight to what matters: you.

In short, our goal is to simplify your experience by creating the smoothest process possible from check-in to check-out.

Hospital and Clinic Forms:

Make your next hospital or clinic visit a little easier by using these printable forms. This is an efficient way to pre-register you or your children if you know that you will be needing treatment in the future.

Patient Consent for Service: Signing of the Patient Consent for Service form is required at every visit for hospital and outpatient services and once a year for clinic visits at CCM Health.

Authorization to Treat Unaccompanied Minors: Use this form to give CCM Health your permission to provide treatment to your child under the age of 18 for any injury or illness that is non-life threatening. For example, your child’s babysitter could present this form to the hospital even if you are unavailable to take your sick or injured child into the hospital. If this form is not presented, a staff member will try to reach you to request verbal authorization to treat your child.

Authorization to Give Unaccompanied Minors Allergy Treatments: Allergy treatments often need to be administered quickly to stop potentially dangerous reactions. If we have this signed and dated form on file, your child can be treated as necessary whether he or she is with you or not.

Authorization for Release of Medical Records: Your medical records are protected by law. This form allows us to release your information from a certain hospital or clinic, or to a certain person who needs them.

Charity Care Application: Are you interested in applying for Charity Care? Fill out this form with the necessary financial information, and we will determine how we can help you.

Proxy Access Request: Link your loved one’s accounts to yours for convenient access to appointments, test results, immunization records, growth charts and more.
